Facial Feminization Surgery:

Here's what Dr. Ousterhout had to tell me about what he'd suggest for me.


SCALP/FOREHEAD:You have a long forehead with some degree of temporal hair loss. I am quite sure that we can advance your scalp into a normal female position. The area of temporal baldness will be reduced but it may not be totally eliminated in one procedure. I can only really stretch the scalp in one direction at a time. If there is some residual or temporal baldness I cannot approach that for at least four months following the original procedure.

Your forehead contour is probably what I call in my classification system a type III. A type III is not better nor worse than a type I or II, it is just for different anatomical situation. The surgical access, black and blueness, pain, time of healing, are basically the same in all of the procedures. In that procedure the entire forehead brow area will be feminized.

CHEEKS:You have nice cheeks now and they are only going to look better as the surrounding areas are reduced and the cheeks become proportionally larger.

NOSE:While you have a nice nose, it is a male nose, and it will need to be reduced. A feminizing rhinoplasty is indicated.

UPPER LIP:Your upper lip is long. When you open your mouth and before you smile, you do not see any of your upper teeth. At your height of 5’81/2” I would want you to show 3 to 4 mm of upper teeth when you open your mouth and before you smile. It has a very feminizing affect. An upper lip reduction is indicated.

CHIN:Your chin is a nice chin but it is obviously a male chin. It is vertically high, it is broad and it may be just a little bit forward. This can all be corrected in an operation we call a feminizing sliding genioplasty.

JAW:You have somewhat of a rectangular face. The angles of your mandible are quite wide and this will all need to be reduced in a procedure called a jaw tapering.

THYROID:Your thyroid cartilage is prominent and you will certainly need a “tracheal shave”.

Therefore, it is my feeling that those procedures that you will benefit from in most of all in feminization of your face will be a scalp advancement, forehead III, open rhinoplasty, upper lip reduction, chin feminization, jaw tapering and a thyroid cartilage reduction. These procedures can all be completed at one time under one general anesthesia in an operation taking approximately 10.5 hours. You will be in the hospital two nights following the surgery. We will want you to spend at least another seven days in San Francisco before returning to Ohio. You will probably want at least another week and a half to two weeks before returning to your usual activities, particularly if it involves being in front of other people.
